The five questions auditors ask about training records

We sit in on a lot of audits. The questions are remarkably consistent, and they are rarely the ones organisations prepare for.

1. Who was assigned this course, and on what basis? Role-based assignment you cannot explain is worse than no assignment.
2. What did the person actually see? If the course was edited after they took it, can you still produce the version they completed?
3. How do you know they understood it? A pass mark on a recall quiz is a weak answer.
4. What happened to the people who failed? Silence here is the most common finding we see.
5. When does this expire, and who is chasing it?

Every one of these is a records question before it is a training question.
